Privacy Collection Statement

Your personal information and that of your nominee and second nominator is being collected by Wyndham City Council for the purpose of considering nominations for the Wyndham Champions of Girl’s and Women’s Sport Program.  By providing the information you agree you have the consent of the nominee and the second nominator for their information to be collected, used and stored for this purpose as specified in the Wyndham Champions of Girls’ and Women’s Sport Nomination Terms and Conditions.  For further information on how personal information is handled, visit Council’s Privacy Policy.

Nominee Declaration


By submitting a nomination to the Wyndham Champions of Girls’ and Women’s Sport Program (‘program’), you agree to be bound by these terms and conditions.

These terms and conditions should be read in conjunction with any relevant program information, e.g. the nomination form and any information published by Council regarding the program.

1.1 To nominate a person for the program, nominators must submit a properly completed nomination form at between 9:00am on 7 August 2020 and 11:59pm on 30 August 2020

1.2 Nominations which are incomplete, do not meet the selection criteria or received outside of this period will not be accepted.

2.1 Nominees must live in Wyndham or play sport or be involved with a sports club within Wyndham.  Council staff, Councillors and their immediate family members are ineligible for nomination.

2.2 Nominators must obtain the nominee’s consent prior to submitting a nomination.  Where a nominee is aged under 18 years, the nominators must have the nominee’s parent/guardian’s consent to submit the nomination.
2.3 Nominators agree to immediately notify Council of any changes to their nomination, including the request to withdraw the nomination, by emailing
3.1 Nominators must have the second nominator’s content to submit their contact details with the nomination.  Evidence of consent must be provided to Council on request.
3.2 Nominators acknowledge and agree that they may be contacted by Council to clarify or provide additional information in connection with their nomination.
Council will nominate all assessments received.  Successful nominees will be contacted and required to confirm their participation in the program.

5.1 Council accepts no responsibility for late, lost, incomplete, invalid or corrupt nominations.

5.2 If for any reason, the program cannot be run as planned, Council reserves the right to cancel, terminate, modify or suspend the program.

5.3 Council’s decisions are final and no correspondence will be entered into.
