My Aged Care is your starting point for accessing Australian Government-funded aged care services. Whether you need help at home, short-term care, or support to enter an aged care home, My Aged Care can assist.

  • Aboriginal and Torres Strait Islander peoples aged 50 and over

Our Community Connectors provide personalised guidance to help residents and carers access the services, groups, and activities they need to stay living independently, active, and connected in the community. We are an access point for Wyndham residents to connect with both formal and informal supports tailored to individual needs.
