Wyndham City Council provides a centralised Information Sharing Request process to support the Child Information Sharing Scheme (CISS) and Family Violence Information Sharing Scheme (FVISS).
These schemes enable authorised organisations to share information to:
- promote the wellbeing or safety of a child
- assess and manage family violence risk
They operate alongside the Multi-Agency Risk Assessment and Management (MARAM) Framework, which supports professionals to identify, assess and manage family violence risk.
Wyndham City Council – Community Support Department’s Prescribed ISE Services
The following Community Support services are prescribed Information Sharing Entities (ISEs):
- Maternal Child Family Health & Well being
- Early Education and Care Services (Kindergarten Services -Educators, Social Workers)
- Supported Playgroups – Family and Sector Partnerships
When to use this form
Use this form when requesting information under:
- Child Information Sharing Scheme (CISS)
- Family Violence Information Sharing Scheme (FVISS)
This includes when you need to:
- request information
- proactively share information
- respond to an information request
This form should be used instead of direct email or informal requests to ensure your request is appropriately recorded, assessed and responded to.
What you will need to provide
To support lawful information sharing, you will be asked to provide:
- Your organisation details (ISE/RAE)
- The Wyndham service(s) you are requesting information from
- Details of the person(s) the request relates to
- The purpose of the request under CISS and/or FVISS
- Relevant context, including well being or family violence risk
- Confirmation that legislative requirements have been met
Providing complete and accurate information will support timely assessment and response.
