Community Aged Care services assist frail aged people who live at home.
Wyndham City Council has 53 Community Aged Care Packages to support people who are eligible for hostel care, but choose to remain at home.
The program includes:
- care management,
- personal care,
- respite care,
- assistance with housework/shopping, and
- delivered meals.
Those intending to use this service need to be seen by the Aged Care Assessment Service.
